Brownlow Health Shortlisted for HSJ Neighbourhood & Community Care Innovation of the Year Award 2026
Brownlow Health is proud to have been shortlisted for the Health Service Journal Neighbourhood & Community Care Innovation of the Year Award 2026 for our pioneering neighbourhood-based model of care, supporting more than 96,000 people across some of our most deprived communities in Liverpool.
By bringing together social prescribing, health and wellbeing coaching, care coordination, proactive outreach, and strong partnerships with community organisations, we’ve aimed to transform access to care for underserved groups including refugees, asylum seekers, vulnerable families, veterans and people experiencing homelessness.
Our approach empowers patients to better understand and navigate healthcare, whilst tackling the wider social determinants of health, and delivering care directly within communities rather than relying solely on traditional clinical settings. The model has generated £57.7 million in social value, released significant GP capacity, supported thousands of patients through preventative interventions, and delivered a remarkable £4.45 return for every £1 invested.
Through our focus on equity, prevention and community-led collaboration, Brownlow Health is demonstrating how integrated neighbourhood care can reduce health inequalities, improve outcomes, and create a more sustainable future for the NHS.
